Open Tournament Report Form

Use this form to report an open tournament to the Board of Directors for inclusion in the state meeting minutes. Please fill out each field in the form. You will recieve an e-mail copy of your tournament submission when the form processes.

• If you are entering a tournament that happens ON ONE DAY ONLY... use the same begin and end date.
• If you are entering a tournament that happens OVER SEVERAL CONSECUTIVE DAYS... use a begin and end date.
• If you are entering a tournament that happens ON SEVERAL NON-CONSECUTIVE DAYS... make a separate submission with this form for each date of the tournament.
